When knitting a sleeve and the pattern says increase both ends in row 19-39 should I increase only on the knit rows? The pattern doesn't specify and I've been told you should usually only increase the knit rows and not the purl rows.

Yes, in general we do increase row on the right side. For the sleeve, usually at the beginning and the end of the same row.
In your case, you will begin increasing from row 19 and then every given row until you get to row 39. Check the pattern to see how far apart the increase should be. (e.g. every 6th row)
